2026 Tri-Wing Encampment — Registration Documents

Important — Read Before Registering

All required forms must be DOWNLOADED, FILLED OUT/SIGNED, and UPLOADED back into Registration Zone BEFORE submitting a registration. Should the number of registered cadet students surpass the number of available slots, the Admin Team will slot cadets based on several criteria. Incomplete or missing forms may impact slotting, so please ensure to download the forms early and upload them prior to the deadline.

Form 1: Parent Signature / OTC Medication Authorization

Encampment Parent Signature / Minor Cadet Over-the-Counter Medication Authorization Form

Required for any cadet who will be under 18 years old at any point during encampment. Cadets who will be 18 during encampment may sign the form for themselves.

Upload to Registration Zone

Electronic copy required before submitting registration.

Bring 1 Hard Copy

Bring a printed signed copy to encampment check-in.

Form 2: TWE 201 Health Disclosure Form

Complete the correct version based on applicant age:

TWE201M

Minor — Under 18 years old

TWE201A

Adult — 18 years old and over

Note on uploading both versions

Both form versions are listed as required on Registration Zone to allow for upload. For the version that does not apply to you, simply upload a blank document to satisfy the requirement.

Example: A 13 year old completes TWE201M and uploads it; they upload a blank document for TWE201A.

Mail Original Signed Copy

In addition to uploading electronically, mail the original to:
